  • Complete vaccination record (Must be professionally translated into English)

Do I need to fill out any forms?

Applicants need to fill out an I-693 form for each applicant.   We also ask applicants need to bring a complete registration package.

Do I need to make an appointment?

Yes, we ask that patients who are looking to have their immigration physical administered to please call our center to confirm which day this service is available.  Our civil surgeon is available by appointments only, seven days a week.

We also advise that you plan accordingly because after receiving the PPD, you must return to our center within 48-72 hours to have it read by one of our nurses.  Failure to do so will result in having to receive another PPD.
